  Insurance Requirements

Tenant COI Requirements

Please ensure the following information has been completed on the tenant COI form:

  • Producer: Name, address and telephone number for the insurance agent.
  • Insured: Name and address of the tenant, subcontractor, architect or general contractor.
  • Companies affording coverage: Name each insurance company providing insurance coverage.
  • Types of insurance: Verify that all coverage levels that apply are marked (e.g. commercial liability, automobile liability, excess liability and worker’s compensation and employer’s liability).
  • Policy number: Different policy number for each insurance type.
  • Policy effective and expiration dates: Coverage dates for the length of work performed.
  • Limits: The maximum insurance limits for each occurrence and in the aggregate afforded by each insurance policy.
  • Combined single limits for bodily injury and property damage:
    • General Liability per each occurrence: $2,000,000
    • Automobile Liability per each occurrence: $2,000,000
    • The above liability coverage can be made up through primary coverage or a combination of primary and umbrella coverage.
    • Worker’s Compensation: State statutory and applicable Federal statutory
    • Employer’s Liability: $1,000,000
    • The “Occur” boxes beneath Types of Insurance must be checked. Checked “Claims Made” boxes will NOT be accepted.
  • Special provisions: The following ownership entity and property management company should appear on the certificate EXACTLY as follows:

    Two North Riverside Plaza Joint Venture Limited Partnership, an Illinois limited partnership, Equity Group Investments, Chai Trust Company, L.L.C., Cushman & Wakefield U.S., Inc., and their respective agents, members, partners, employees, officers, directors, shareholders and lenders.

    Certificate Holder: (MUST READ AS FOLLOWS)
    Two North Riverside Plaza Joint Venture Limited Partnership
    Cushman & Wakefield U.S., Inc.

  • Broker MUST sign certificate.
  • Cancellation should specify 30 days written notification prior to cancellation.

Vendor COI Requirements

GENERAL LIABILITY $2,000,000 Per Occurrence
AUTOMOBILE LIABILITY $2,000,000 Per Occurrence
The above can be made up through primary coverage or a combination of primary and umbrella.
WORKERS COMPENSATION & EMPLOYERS LIABILITY
Statutory

$1,000,000 Per Occurrence
$1,000,000 Each Employee/Disease
$1,000,000 Disease/Policy Limit
CANCELLATION NOTICE 30 Days

Note: All General Liability policies MUST identify and read as follows:
